Untuk memperoleh rekomendasi sebagaimana dimaksud dalam Pasal 4 ayat (3) pemohon mengajukan permohonan secara online dan/atau langsung (manual) kepada Direktur Kesehatan Masyarakat Veteriner dan Pascapanen dengan tembusan Direktur Jenderal Peternakan dan Kesehatan Hewan, sesuai dengan Format-1.
Pasal 23
(1) Permohonan Rekomendasi oleh Pelaku Usaha, Badan Usaha Milik Negara, dan Badan Usaha Milik Daerah harus diajukan pada tanggal 1-31 Desember tahun sebelumnya, tanggal 1-31 Maret, tanggal 1-30 Juni, dan tanggal 1-30 September tahun berjalan.
(2) Permohonan Rekomendasi oleh Lembaga Sosial, dan Perwakilan Negara Asing/Lembaga Internasional dapat diajukan sewaktu-waktu sesuai dengan kebutuhan.
(3) Dalam hal untuk menjaga ketahanan pangan dan stabilitas harga, Badan Usaha Milik Negara dapat mengajukan permohonan Rekomendasi sebagaimana dimaksud pada ayat (1) sewaktu-waktu, sesuai penugasan dari Menteri Badan Usaha Milik Negara.
Pasal 24
(1) Permohonan Rekomendasi yang diajukan oleh Pelaku Usaha, Badan Usaha Milik Negara, dan Badan Usaha Milik Daerah harus dilengkapi persyaratan:
a. Kartu Tanda Penduduk (KTP) dan/atau identitas pimpinan perusahaan;
b. Nomor Pokok Wajib Pajak (NPWP);
c. Surat Izin Usaha Perdagangan (SIUP);
d. surat tanda daftar atau izin usaha di bidang peternakan dan kesehatan hewan;
e. akta pendirian perusahaan dan perubahannya yang terakhir;
f. Nomor Kontrol Veteriner (NKV);
g. penetapan sebagai Importir Terdaftar (IT) produk hewan;
h. surat keterangan bermaterai kepemilikan tempat penyimpanan berpendingin (cold storage) dan alat transportasi berpendingin disertai bukti/dokumen pendukungnya, kecuali untuk pemasukan daging olahan siap saji yang tidak memerlukan fasilitas berpendingin sebagaimana informasi pada label produk;
i. rekomendasi dinas provinsi;
j. mempunyai dokter hewan yang berkompeten di bidang kesehatan masyarakat veteriner, dibuktikan dengan surat pengangkatan atau kontrak kerja dari pimpinan perusahaan;
k. laporan realisasi pemasukan periode sebelumnya;
l. bukti penyerapan daging sapi lokal telah diverifikasi oleh dinas provinsi dan/atau kabupaten/kota asal daging sapi lokal; dan
m. surat pernyataan bermaterai yang menyatakan bahwa dokumen yang disampaikan benar dan sah.
(2) Permohonan Rekomendasi yang diajukan oleh Lembaga Sosial harus dilengkapi persyaratan:

Article 23

e. certificate of incorporation and the latest amendment;
f. Veterinary Control Number (NKV);
g. determination as a Registered Importer (IT) animal products;
h. certificate of ownership stamped refrigerated storage (cold storage) and refrigerated transportation equipment with evidence / supporting documents, except for the inclusion of processed meat fast food that does not require refrigerated facilities as well as information on the product label;
i. recommendations provincial agencies;
j. have veterinarians who are competent in the field of veterinary public health, evidenced by a letter of appointment or employment contract of the head of the company;
k. realization of prior period income statements;
l. evidence of absorption of local beef has been verified by the provincial office and / or district / city local beef origin; and
m. a letter stamped declaration stating that the submitted documents and valid.
